Completing the initial paperwork
All new patients are required to fill out a confidential intake form and stress questionnaire. This allows us to fully understand your past and present state of health. As the forms are quite comprehensive, they usually take 15-20 minutes to complete. You can save time by downloading these forms and complete them prior to coming to our office for your initial appointment.

If you have any previous x-rays, CT scans, MRI, ultrasound, etc. please bring any reports with you. Also a list of any medications that you may be taking should be made available.
Additionally, we can do a courtesy verification of your insurance health benefits upon request (please inform the front desk staff and provide your insurance information).
Consultation & Examination
You will have your first consultation with the doctor. At this time you will discuss your health concerns and goals.
You will then undergo an in-depth examination to determine if you may be helped by chiropractic care. This comprehensive assessment detects the presence of SUBLUXATIONS (spinal joint dysfunction causing nerve interference). Since your nervous system controls every single tissue and organ system in your body, interference in the nerve flow would prevent your body from working properly and can result in various unwanted health problems.
The exam procedures consisting of a computerized postural evaluation, range of motion testing, orthopedic testing and computerized spinal scan will allow the doctor to assess the health of your spine and the associated nerve flow to the rest of your body. If indicated, x-rays will be taken to visualize the location and extent of subluxation in your spine, as well as how your body is reacting to the subluxation.
Please note that no treatment will be rendered on the first visit. It is crucial to your well-being that the doctor has time to review your case thoroughly in order to determine the appropriate recommendations to facilitate your recovery.
Your first visit will be approximately 60-75 minutes in duration. At the completion of this initial visit, we will book a second visit to go over the Doctor’s Report of Findings (examination results and recommendations).
